Summary: Longmont United Hospitals Medical Oncology team is looking for vacation coverage for the December/January shifts detailed below. This opening is currently only for these December shifts. If the team like the provider, we also have a need open starting January 6th that would be an option but not a requirement. The Hope Cancer Center is a new Hematology/Oncology clinic with a dedicated chemotherapy treatment space for patients t receive their cancer treatment. Coupled with our Breast Center, breast surgery capabilities, and our external beam treatments in radiation oncology, this role will be responsible for the treatment of hematologic diagnoses and medical oncology diagnoses in our cancer center. Physician will be responsible for clinic coverage and chemotherapy supervision 4 days per week. As a new clinic, this role is also responsible for building/growing this subspecialty practice.

Rate Caps for Submission:

• Malpractice Must be included in rates.
• Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
• Hourly xx595 and below - Per hour, not OT
  
• Holiday Must be 1.5x the hourly rate. Per holiday hour, no OT

• All shifts are 7a-5p with 30 minute unpaid lunch.

EMR Required Training:

• Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
• EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.

• Board Certified Medical Oncology - Required
• Active CO license - Required or IMLC (LOQ in hand at time of submission)

• Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
• Candidate must be comfortable and willing to see Hematology patients - Required - Must note at name clear
• Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
• Candidate must be available for any of the dates week stints detailed in job description to be considered - Required
• Clean MP/background - Required for quick credentialing, site will place priority for candidates with a self inquiry NPDB at time of presentation.
