Summary: St. Anthony Hospitals Radiation Oncology team is looking for a locum provider to help with coverage gaps WedFri starting Jan 22 through May 2. Shift includes 8a5:30p WedFriday.

BID DUE DATE: Dec 6th at 5p EST!

Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly xx395 and below Per hour not OT

Holiday Must be 1.5x the regular rate. Per holiday hour no OT

Details:

• Facility Location: 11750 W 2nd Pl Suite 150 Lakewood CO 80228
• Setting: Outpatient
• FTE: 0.675
• Days per month: 3 shifts per week about 12 shifts per month dependent on the number of weeks per month.
• Required Procedures: Radiation Oncology Consultations/New Patient Visits Follow ups Established Patient Visits On Treatment visits Treatment Planning Treatment Supervision.
• EMR: EPIC Aria
• Machine is a Varian True Beam. Treatment planning is done in Eclipse.
• Support Staff: RN
• Reason for Coverage: Gaps in coverage.
  

EMR Required Training:

• Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
• EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.

Travel Preferences:

• Preferred Lodging: Preferred hotels
• Air Travel: Local drive in candidates only.
• Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Credentialing Information:

• Timeframe: 6090 days is standard.
• Emergency temp privs Yes with clean file only. 1 week minimum needed to complete file.

• Billing enrollment Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less No

• What is the makeup of your support staff 2 Radiation Therapists 1 CT Sim Radiation Therapist 2 RNs (1 on Monday and Friday) 1 PSR (Registration/Scheduling)
• I see 1 FTE is being asked Willing to use 23 providers Preference will be given to single providers who are able to cover a majority of the dates.

Requirement description :

• Board Certification Radiation Oncology Required
• Active CO license Required or IMLC (LOQ in hand at time of submission)
• Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
• Local drive in candidates only Required
• Site willing to accept 2 providers to fill the need candidates must be available WedFri for the weeks available. Candidate must confirm wedfri availability and state the specific weeks they are able to offer between the dates need Required candidates who are able to cover the full stint every week will get priority over those not able to fill all the weeks.
• Clean MP/background Highly Preferred site will place priority for candidates with a self inquiry NPDB at time of presentation.
