Are you a compassionate and dedicated mental health professional looking to make a real impact in the lives of individuals living with serious mental illness?

We are seeking a Mental Health Practitioner to join our Assertive Community Treatment (ACT) team. In this role, you will provide comprehensive, in-home support to individuals in need, helping them achieve their recovery goals.

What you'll do:

Develop and implement individualized treatment plans.
Provide support and guidance to individuals in their homes and communities.
Assist with daily living skills, such as cooking, cleaning, and budgeting.
Connect individuals with essential resources, including housing, employment, and healthcare.
Advocate for your clients and build strong relationships with their support systems.
Participate in team meetings, case conferences, and on-call rotations.
