We are seeking a compassionate and dedicated Mental Health Practitioner to join our Assertive Community Treatment (ACT) team. In this impactful role, you will provide comprehensive, individualized support to adults living with mental illness and substance use disorders.

What You'll Do:

Provide direct clinical services: Conduct assessments, develop and implement treatment plans, and provide individual and group therapy.
Assist clients with daily living skills: Help clients with tasks such as housing, employment, and daily living skills (e.g., cooking, shopping, budgeting).
Coordinate care: Collaborate with other members of the treatment team, including psychiatrists, nurses, and peer specialists, to ensure seamless and coordinated care.
Provide crisis intervention: Respond to client crises and provide support during times of need.
Advocate for clients: Assist clients in accessing benefits, housing, and other essential resources.
Engage with the community: Build relationships with community partners and advocate for the needs of individuals with mental illness.
What You'll Bring:

Master's degree in Social Work, Counseling, or a related field.
2,000 hours of supervised experience working with individuals with mental illness.
Strong clinical skills, including assessment, diagnosis, and treatment planning.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work independently and as part of a team.
Passion for helping others and a commitment to providing high-quality care.
