Services Coordinator - DTA

This Services Coordinator – DTA is responsible for ensuring high quality of services are provided to individuals with physical and/or intellectual challenges by monitoring the services provided, developing staff training and opportunities, assisting in the development and implementation of financial, strategic and programmatic goals, ensuring contract compliance and fulfilling The Opportunity Tree's mission, vision, and values.

  • Conduct tours, coordinate intakes and complete initial evaluation of persons referred to and accepted by The Opportunity Tree.
  • Responsible for accurate and timely completion and reporting of agency attendance tracking systems.
  • Complete skill and behavioral assessments as well as evaluations on an annual basis.
  • Develop, implement, and evaluate Individual Service Plans (ISP).
  • Create goals and plans to facilitate acquisition of life skills and vocational skills
  • Represent The Opportunity Tree as part of the IDT for the members' ISPs.
  • Manage time and multiple projects and duties effectively and accurately.
  • Pro-actively maintain communication with families/guardians of Members to ensure success in programs leading to progressive movement as appropriate.
  • Oversight of Member's The Opportunity Tree program including plan development, implementation and on-going evaluation.

Requirements:
  • Minimum of 18 years of age
  • Must be fully vaccinated against COVID-19.
  • Current driver license with Non-Restricted Motor Vehicle Record and compliance per the requirements of The Opportunity Tree's insurance carrier.
  • Excellent communication skills, including the ability to write, speak, and listen effectively in English.
  • Experience: Two (2) years in the Intellectual and Developmental Disabilities (IDD) field in a vocational, rehabilitation, or similar setting.
  • Education: Bachelor degree in Human Services preferred.
  • Knowledge: Intermediate in MS Outlook, MS Word, MS Excel, web browsing.
  • Knowledge of best practices in adult day programs and vocational training programs.
  • The ability to utilize data to assist in making financial, strategic, and programmatic decisions.
  • Personal enthusiasm for participation in a non-profit organization serving individuals with intellectual and developmental disabilities.
  • Successful completion and maintain all required training which may include CPR, First Aid, and Prevention and Support.
  • Successfully meet AZ-DPS requirements for Fingerprint Clearance Card.
  • Reliable transportation and travel for The Opportunity Tree business.
  • One year of supervisory skills to include but not limited to knowledge of management principles in staff interaction and task productivity, leadership abilities, problem identification and resolution administration.
